Since opening our services Elevate Supported Living have developed a wide range of therapeutic services for citizens with mental health, complex needs, autism and learning disabilities. We work in partnership with local authorities to provide sustainable housing with support and reduce hospital readmissions through our services. We strive to deliver and maintain outstanding services throughout, with a key focus on internal systems of quality care, treatment and delivering positive outcomes.
Our low, medium and high support services are therapy and recovery focused, with the aim to effectively progress service users through secure care and into the least restrictive environment possible. With a focus to reduce the possibility of re-admission back into secure hospitals.
We provide a person centred, therapeutic, progressive and supported model of care that incorporates crisis houses to prevent hospital admissions, recovery houses that enable a smooth transition from hospitals into supported living and a wide range of supported living homes that meet individual needs within the community.
